Rethinking Psychiatry

Myth

"Psychiatry is only about talking. You don't need blood tests for mental health problems."

Fact

"Modern psychiatry combines clinical expertise with lab science to find accurate, treatable causes."

Myth

"If tests are normal, your symptoms must be imaginary or psychological."

Fact

"Normal results help guide the treatment approach. Symptoms are always real. Tests help us understand why."

How Doctors Think — Diagnostic Logic

Every symptom follows a clinical pathway. Diagnostic tests confirm or rule out each step.

🌙 Depression Pathway

1
Patient reports low mood, fatigue, hopelessness
Initial clinical assessment
2
Blood tests ordered
CBC, Thyroid (TSH/T3/T4), Vitamin B12, D3
3
Results reviewed
Identify any abnormal values
🔴 Abnormal → Treat physical cause first (e.g. B12 injection, thyroid medication)
🔵 Normal → Begin psychiatric / psychological treatment pathway

💓 Anxiety Pathway

1
Palpitations, panic, chest tightness
Rule out cardiac and thyroid first
2
ECG + Thyroid panel ordered
Electrolytes, Cortisol if needed
3
Cardiac arrhythmia ruled out
Hyperthyroidism confirmed or excluded
🔴 Abnormal → Cardiology / Endocrinology referral
🔵 Normal → Anxiety disorder diagnosed, therapy + medication

Structured Testing — Level by Level

Tests are ordered based on your symptoms, not just guesswork. Start with basics, escalate only when needed.

🩸
CBC (Complete Blood Count)
Detects anaemia, infections, inflammation that can mimic depression & fatigue
🦋
Thyroid Panel (TSH, T3, T4)
Thyroid imbalance is one of the most common reversible causes of anxiety & depression
💊
Vitamin B12
Especially critical in Kota's vegetarian population. Low B12 causes brain fog & low mood
☀️
Vitamin D3
Widely deficient in India. Linked to depression, fatigue, and cognitive decline
🍬
Blood Sugar (Fasting + PP)
Hypoglycaemia and diabetes can cause mood swings, anxiety, and confusion
⚖️
Serum Iron / Ferritin
Iron deficiency is a leading cause of restlessness, fatigue, and poor concentration
ECG (Electrocardiogram)
Rules out cardiac causes of palpitations and panic attacks before psychiatric diagnosis
🧪
Electrolytes (Na, K, Ca, Mg)
Imbalances cause confusion, muscle weakness, and mood disturbances
🫁
LFT — Liver Function Test
Hepatic encephalopathy can cause psychiatric symptoms. Required before medications
🫘
KFT — Kidney Function Test
Required for safe dosing of lithium and other psychiatric medications
😴
Cortisol (AM/PM)
Elevated cortisol in chronic stress linked to anxiety, insomnia, and depression
🧲
Folate (Folic Acid)
Folate deficiency linked to depression and poor medication response
🧠
MRI Brain
Rules out tumours, white matter lesions, and structural causes of psychiatric symptoms
EEG (Electroencephalogram)
Detects seizure disorders that can present as psychiatric episodes
🔬
Toxicology Screen
Rules out substance use or accidental medication poisoning causing symptoms
🧬
Genetic Testing (Pharmacogenomics)
Identifies how your body metabolises psychiatric medications for personalised dosing
🔥
Inflammatory Markers (CRP, ESR)
High inflammation is increasingly linked to treatment-resistant depression
💉
Lithium Serum Levels
Critical safety monitoring for patients on lithium therapy — toxicity can be fatal

"Not All Mental Symptoms
Are Psychiatric"

Many people are treated for years for a psychiatric disorder when the true cause is physical — and completely treatable.

🦋
Thyroid Disorders
Both hypothyroidism and hyperthyroidism cause anxiety, depression, and psychosis-like symptoms
✓ Fully Reversible
💊
Vitamin B12 Deficiency
Brain fog, irritability, and psychosis — especially in Kota's vegetarian student population
✓ Fully Reversible
☀️
Vitamin D Deficiency
Strongly linked to depression, fatigue, and poor motivation. Very prevalent across Rajasthan
✓ Fully Reversible
🩸
Anaemia
Low haemoglobin causes fatigue, concentration problems, and depressive symptoms
✓ Fully Reversible
💉
Medication Side Effects
Many common drugs (steroids, blood pressure meds) cause mood changes and anxiety
✓ Manageable
🔥
Infections & Inflammation
Viral encephalitis, autoimmune conditions can present with psychiatric features
✓ Treatable
✨ Correcting these physical causes can completely reverse psychiatric symptoms — without long-term medication.

Dementia vs Delirium — Know the Difference

One is gradual. The other is a medical emergency. Every minute matters.

Feature Dementia 🧠 Delirium ⚡
OnsetSlow (months to years)Sudden (hours to days)
CauseProgressive brain degenerationUnderlying medical emergency
ConsciousnessUsually intactFluctuates, often impaired
ReversibilityUsually not reversibleOften fully reversible if treated
Common TestsMRI, cognitive tests, B12Electrolytes, glucose, ECG, infection markers
UrgencyModerate — outpatient workupEMERGENCY — hospitalise immediately
🚨 Sudden confusion, especially in elderly patients, is a medical emergency. Go to hospital immediately — do not wait.

Why Local Context Matters in Diagnosis

Kota has unique health patterns that directly influence which tests are most important.

🏙 Common Health Patterns in Kota

  • B12 Deficiency — Very high among vegetarian students and families. Often missed.
  • Vitamin D Deficiency — Paradoxically common despite high sun exposure in Rajasthan due to indoor study lifestyles.
  • Student Stress Syndrome — Kota's coaching culture creates extreme chronic stress, elevating cortisol and anxiety.
  • Iron Deficiency — Especially in adolescent girls and young women.
  • Sleep Disorders — Disrupted sleep from late-night study is now reaching clinical concern levels.

🤝 Addressing Cultural Barriers

  • "Why blood tests for a mental problem?" — Because physical body is part of the mind. Both must be examined.
  • "We don't believe in psychiatry" — Our approach is grounded in medical science, not stigma.
  • "Tests are expensive" — Basic panels start at ₹500. Treating the wrong cause costs far more.
  • "We tried homeopathy" — We respect your journey. Our tests help find what's actually causing symptoms.
  • "He'll be labelled" — Patient privacy is strictly maintained. Reports are confidential.

When Tests Changed Everything

These real-world examples show why diagnostic testing matters in psychiatry.

Case 01
The Student Who Couldn't Study
📚 19-year-old male, Kota coaching student
6 months of fatigue, brain fog, poor grades
Referred for "depression" treatment
Blood test: Vitamin B12 = 89 pg/mL (severely low)
B12 injections prescribed for 4 weeks
✨ Full cognitive recovery. No psychiatric medication needed. Back to top of class.
Case 02
The "Anxiety" That Was Actually Thyroid
👩 32-year-old female, homemaker, Kota
Panic attacks, palpitations, weight loss
Already on anti-anxiety medication for 8 months — no improvement
Thyroid test: TSH suppressed (0.01) — Hyperthyroidism confirmed
Referred to endocrinologist, anti-thyroid treatment started
✨ All anxiety symptoms resolved within 3 months. Anti-anxiety medication stopped completely.

Medication Safety & Monitoring

Some psychiatric medications require regular monitoring to ensure your safety.

⚠️

Lithium Toxicity

Lithium has a narrow safe range. Serum levels must be checked every 3–6 months. Toxicity can be life-threatening.

🫀

Metabolic Monitoring

Antipsychotics can affect weight, blood sugar, and cholesterol. Regular CBC, glucose, and lipids are essential.

🫁

Liver Function (LFT)

Mood stabilisers and antidepressants are processed by the liver. LFT must be checked before starting and periodically.

🫘

Kidney Function (KFT)

Several medications are renally cleared. KFT protects against accumulation and toxicity, especially in elderly patients.

Frequently Asked Questions

Why would I need an ECG for anxiety?
Palpitations and chest tightness can be caused by both anxiety AND heart conditions. An ECG rules out arrhythmia (irregular heartbeat), tachycardia, and other cardiac causes before a psychiatric diagnosis is confirmed. Treating anxiety when the real cause is cardiac can be dangerous.
Do I need to fast before my blood tests?
Yes, for blood sugar and lipid panels, 8–12 hours of fasting is required. For thyroid tests and vitamin levels, fasting is preferred but not always mandatory. Your doctor will specify. Morning appointments are ideal. You may drink plain water during the fasting period.
What if all my tests come back normal?
Normal results are valuable information — they are not a sign that your symptoms are imaginary. Normal physical tests mean we can confidently pursue a psychological pathway for treatment. Your symptoms are real. Tests help us understand them better.
Can tests actually diagnose schizophrenia or bipolar disorder?
No — these diagnoses are clinical and made through assessment. Tests do NOT diagnose schizophrenia or bipolar disorder. Their role is to RULE OUT physical causes that can mimic these conditions. This is a critical distinction: tests guide and confirm, they do not replace the psychiatrist's clinical judgment.
When should I time my lithium test?
Lithium serum levels should be tested 10–12 hours after your last dose (trough level). Typically, this means testing in the morning after your previous evening's dose. Do not skip your dose before testing unless specifically instructed. Results outside the 0.6–1.2 mEq/L range need immediate medical attention.
Is Vitamin D deficiency really common in sunny Rajasthan?
Yes — surprisingly so. Despite the high sun intensity, most students and professionals in Kota spend 6–10 hours daily indoors (classrooms, coaching centres, offices). Skin coverage through clothing, sunscreen use, and high melanin in darker skin also reduce synthesis. Studies show 70–90% of Indians have suboptimal Vitamin D levels.

Get the Right Diagnosis. First.

Stop guessing. Stop the wrong treatment. A single blood test could change everything — and it starts with one appointment in Kota.
